The “Bone & Joint Journal”, world renowned scientific journal in the area of orthopaedics, has recently published an article by Paulo Rego entitled “Femoral head reduction osteotomy with simultaneous periacetabular osteotomy for severe femoral head deformities” . This is an essential and innovative surgical approach, the article reporting the results of research on the treatment of patients with severe femoral head deformities, caused by sequelae of childhood hip diseases. Although there being several surgical techniques, the combination of femoral head reduction osteotomy (FHRO) and periacetabular osteotomy (PAO) represents a significant advancement in the treatment of complex cases, that conventional methods may not resolve effectively. This double technique aims to enhance articular congruence and restore the hip mechanics, providing a substantial improvement of the joint performance in daily activities. In the article, the author describes in detail his surgical methodology and the clinical results obtained with a minimal follow-up of the patients along two years. Considering the complexity of the deformities, the article opens new perspectives for future research and innovations in surgical techniques based in the vascular anatomy of the femoral head and ultimately aiming at the improvement of radiographic results and the quality of life for adolescent patients.
