Hospital da Luz Coimbra performed recently the first biportal endoscopic spine surgery, an innovating approach using two portals for visualization and direct access to the spinal canal and associated structures, allowing to complete complex procedures causing less trauma and pain, which reflects in a faster recovery of the patient. “This technique has revolutionized the treatment of many spine diseases, namely lumbar disc herniations, lumbar spinal stenosis, and cervical disc herniations”, explains Ricardo Frada , the orthopaedist who conducted this surgery. Biportal endoscopy may also be used in combination with other spinal fusion surgeries, so as to optimize results and minimize trauma. In selected cases of patients previously submitted to spine surgeries, it may offer a less invasive option for revision procedures, thus reducing the risks associated with reoperations. Among the advantages of biportal endoscopic spine surgery for patients and surgeons, we highlight the following: Contrarily to conventional surgery, this is a minimally invasive procedure only requiring small incisions. Therefore, post-operatory pain reported by patients is minor, just as the time of recovery; It allows the surgeon direct high-resolution visualization of the vertebral and neurological structures, enabling surgical procedures of high precision and safety; Being minimally invasive, the potential post-operatory complications are also reduced , when compared with traditional open surgery methods, in terms of infection, excessive blood loss, hematoma and nerve lesions; Shorter hospitalization time and quicker return to daily activity; Besides the abovementioned spine pathologies, it can be used in more complex decompression interventions , with the possibility of association to spinal fixation. “Considering its numerous advantages and specific applications, this technique is transforming the treatment of many spine pathologies, becoming an essential tool in modern orthopaedic and neurosurgical practice. Its continuing evolution promises the additional improvement of results and extends its applicability, benefiting primarily the patient”, stresses Ricardo Frada.
